Die Stadt Otočac liegt am Fluss Gacka, im westlichen Teil des Karstfeldes der Gacka (Gacko polje) zwischen dem Velebit-Gebirge und der Mala Kapela. Die Stadt selbst teilt sich in eine Unterstadt und eine Oberstadt.
Zu den Highlights in der Region zählen das Höhlensystem Bezdanjača am Berg Vatinovac und das Velebit-Bioreservat.
